Consultation forms, such as consent forms for procedures or medications, health and vaccination certificates, and discharge instructions, are commonly used by animal clinics and hospitals for various standard documentation needs.
To add a consultation form to a consultation:
During an active consultation, open the forms section:
New consultation page: in Consultation overview, select Show forms. The Consultation forms section opens.
Old consultation page: go to the Consultation details section.
Add the form:
New consultation page: select + Add form, then select the form from the list.
Old consultation page: in the Forms section, select a form from the Add drop-down list.
In the preview dialog that opens, fill in the form.
Select Save changes.

After you close the preview, the form is listed with the consultation's other forms. All saved consultation forms are also accessible through the History tab on the client or patient page when the Forms filter is selected.
